If we do not take your insurance, we offer out-of-network care.
Out-of-network rates:
45 minute appointments:
Physical Therapy Evaluation: $125
Physical Therapy Treatment: $115
Physical Therapy Treatment Package: $399 for 4 visits (visits must be used within 1 year of purchase)
1 hour appointments:
$130 for evaluations and follow-ups
If requested, we can provide you with a Superbill that you can submit to your insurance for any available out-of-network benefits.
