In-Network

We are currently in-network with the following insurance plans:

  • Horizon Blue Cross Blue Shield of NJ

  • Other Blue Cross plans in the BlueCard network

  • Medicare Part B (not Advantage plans)

Out-of-Network

If we do not take your insurance, we offer out-of-network care.

Out-of-network rates:

45 minute appointments:

  • Physical Therapy Evaluation: $125

  • Physical Therapy Treatment: $115

  • Physical Therapy Treatment Package: $399 for 4 visits (visits must be used within 1 year of purchase)

1 hour appointments:

  • $130 for evaluations and follow-ups

If requested, we can provide you with a Superbill that you can submit to your insurance for any available out-of-network benefits.

Not only can our exercise prescription help you return from injury, we can also help you improve your performance in the gym, in your sport, or with daily life tasks. We’ll help you get stronger, move more efficiently, and improve your fitness.
